Investigation into Police Contact Following Fatal Explosion

The Independent Office for Police Conduct (IOPC) is investigating the interactions between Avon and Somerset police and Jo Shaw and her former partner after both died in an explosion on 3 May in Bristol.

Jo Shaw, aged 35, was killed in the blast at a property in the Frenchay area of the city. Her ex-partner, Ryan Kelly, 41, also died in the explosion.

The IOPC confirmed it is examining the contact both individuals had with Avon and Somerset police. The last police contact with Shaw occurred four days before the explosion. The investigation will review incidents from May 2017 to May 2022. Kelly had been previously accused of stalking, harassment, and assault.

"My thoughts and sympathies are with everyone affected at this very difficult time," said Derrick Campbell, IOPC engagement director.
"We are independent of the police and it is important that we carry out a thorough investigation, to fully establish the circumstances of what happened prior to the tragic events."

Police Response and Incident Details

Avon and Somerset police are treating the explosion as deliberate and have recorded Shaw’s death as a homicide. The force has stated that it is not seeking anyone else in connection with her death.

Three other individuals, including a child, sustained minor injuries in the incident.

At Avon coroner’s court, it was reported that Shaw died from a "penetrating fragmentation blast injury to the chest and pelvis," while Kelly suffered "blast injuries to head, neck, torso and limbs."

Police received a call at 6:17am from a person inside the house reporting a domestic-related incident, stating that a man known to them had forced entry.

The caller remained on the line as officers were dispatched. At 6:30am, the caller informed police that the man was believed to be in possession of an explosive device.

Approximately two minutes later, an explosion occurred at the property, resulting in the deaths of Shaw and Kelly.

Officers arrived at the scene at 6:34am, with fire and ambulance services arriving shortly thereafter.

Aftermath and Safety Measures

A major incident was declared following the explosion. Police collaborated with Bristol City Council to evacuate nearby residents and establish a rest centre.

Searches were conducted by an army explosive ordnance disposal team and specialist police officers.

Additional searches took place at an address where Kelly resided; no evacuations were necessary at that location.
